Role Description
This is a part-time on-site role for a Licensed Practical Nurse. The LPN’s day-to-day tasks will include providing direct at home patient care, monitoring patient conditions, documenting patient progress, and collaborating with other healthcare professionals to improve patient outcomes. The LPN will be working in the Hampshire County and be available 24-30 hours a week.

Qualifications
Active LPN License
5 years of experience working with elders
Experience in long-term care or a related field
Excellent communication and interpersonal skills
Ability to work flexible hours and adapt to changes in patient assignments
Willingness to work in a fast-paced environment
Attention to detail and strong organizational skills
Experience working with electronic medical records is a plus
Willingness to receive training in areas such as dementia care and fall prevention
Ability to be on-call
Competitive salary dependent on experience, SIMPLE IRA, Vacation time
Must have a reliable car, insurance, CPR and LPN documentation
