At Elder Care Access, our mission is to ensure you and your loved ones feel prepared, supported, and empowered while navigating to process of aging.
Led by Geriatric Care Manager Sheryl Fappiano, LSW, CMC, our team includes on-call nurses, home safety experts, and care planners who know how to simplify complex decisions. We provide guidance on community resources to help families save money, improve safety, and secure the best possible care across a variety of environments.
Our work goes beyond planning, we are long-term partners in care. From coordinating ongoing services to attending doctor’s appointments, tracking medications, and advocating with medical teams, we make sure your voice is heard and your needs are met. We also help with transitions, offering housing alternatives, overseeing care at home, and being there in times of change or crisis.
For us, this work is a calling. Every client is treated with dignity, respect, and the same individualized attention we would give our own parents.
Our Caregivers
Elder Care Access has built a team of several dedicated caregivers that have worked with our clients for years. Elder Care Access is incredibly proud that the work we do for seniors has inspired our caregivers to remain with us. Sheryl Fappiano LSW, CMC
Member of the National Association of Professional Geriatric Care Managers. Member Aging Life Care Association.

Sheryl Fappiano is a professional Geriatric Care Manager (GCM) and licensed social worker with over 41 years of experience working with seniors in various capacities such as home health aide, certified nursing assistant, case manager for a local area agency on aging, medical assistant).

Sheryl began Elder Care Access because she recognized a need in the community for a trained professional to assist families with the many issues, struggles, and unknowns around senior care. Since becoming a licensed social worker, she has always felt passionate about advocating for seniors, especially those who are vulnerable.
​
After opening Elder Care Access in 2003, Sheryl gravitated towards working with seniors who are vulnerable in the community and those that have no family close by to assist with setting up services to help them age gracefully in place.
​
Working closely with local guardians, families, protective services, and home care agencies has become her passion.
Sheryl has created a team of professionals that works diligently and lovingly to honor the wishes of her clients.


Abby Mish

Office Manager
Abby is Elder Care Access's amazing Office Manager. She brings over 20 years of experience in Administration, Finance, and HR, along with her deep love for the elderly and strong belief in the mission of Elder Care Access. Abby is passionate about keeping our operations running smoothly and efficiently and is always ready to support families as they navigate care needs. She also assists clients with Long-Term Care Insurance questions and processes, helping to make this part of planning more approachable and manageable.

Kathy Hunter
RN/GCM
Kathy has been a registered nurse since 1993, graduating from St Joseph College with her BSN, RN. She worked at Fairview Hospital in Great Barrington MA for 1 year before entering the field of home health care in 1994. Over the past 30 years she has worked in various roles as a nurse case manager in the homecare field as well as in the hospital setting. She has held many positions in nursing intake, quality and supervisory roles. She is passionate about working with the Geriatric population in particular and has a strong desire to be the voice and advocate for all those in need. Kathy is honored to be a part of the Elder Care Access family and is excited to be representing and supporting all of her clients.

Lisa Safron
RN/GCM
Lisa has been a registered nurse since 1992, graduating Elms College with her BSN, RN. She began her nursing career at Baystate Medical Center holding many roles during her tenure as charge/triage nurse, and staff nurse in addition to being a focus group member for geriatric and respiratory patients and their families. In 2001 her journey switched to school nursing and held many different roles in the school system such as administrator, asthma data research and direct care. Although these arenas may seem different, Lisa continues to allow her root belief system to guide every role, that of advocacy, a holistic approach to care and finding a voice for those who may not feel heard. She is also a Reiki Master and co-facilitated monthly Reiki nights at the Cancer Connection.

Alli Zadworny
LPN
Alli Zadworny brings a strong background in nursing and a deep passion for helping others. She studied nursing at Elms College before graduating from GCC in 2022. Her experience spans wellness nursing at Armbrook Village, agency work in nursing homes, and addiction care—each role reinforcing her dedication to advocacy and compassionate care.
With a foundation in childcare and years of hands-on nursing experience, Alli is honored to be part of Elder Care Access, where she continues to support and advocate for those in need.

Courtney Fappiano
Marketing and Intake Manager
Courtney oversees marketing and client intake at Elder Care Access, helping families find the support they need with compassion and clarity. With a background in communications and a deep commitment to serving others, she ensures every client’s journey begins with confidence and a smile.

Kaylie Nareau
Office Administration Assistant
Kaylie is an Administrative Assistant at Elder Care Access, and enjoys helping the office run smoothly for our clients and their families. From scheduling and calls to staying connected with agencies, she is here to support and make the process a little easier.
Outside of work, she loves spending time in nature—hiking, walking, and taking in the beautiful views New England has to offer. She is also a big fan of camping, weightlifting, reading, cooking, and making memories with my family and friends.
