Licensed Practical Nurse (LPN) 

Position Details:

  • Location: Hampshire County, MA

  • Position Type: Part-time, Hybrid, 10-20 hours a week

  • Compensation: $31/h

Role Overview 

We are seeking a Licensed Practical Nurse (LPN) to join our dedicated team. This role involves providing direct, hands-on care to clients, monitoring their health conditions, maintaining detailed documentation, and collaborating closely with other healthcare professionals to ensure optimal outcomes. The ideal candidate is a compassionate, experienced nurse who values teamwork, independence, and flexibility in their workday. 

This position is ideal for a seasoned LPN who wants to maintain meaningful work while scaling back from a full-time schedule. You’ll enjoy the flexibility, autonomy, and personal connections that come from working with a small, mission-driven organization that values compassion as much as clinical skill.

Key Responsibilities 

  • Provide direct nursing care and monitor client health and wellbeing. 

  • Document patient progress accurately and maintain electronic medical records. 

  • Collaborate with physicians, caregivers, and community partners to coordinate care. 

  • Identify changes in patient conditions and communicate concerns promptly. 

  • Participate in ongoing training, including dementia care and fall prevention. 

  • Be available for on-call rotation two weekends per month. 

  • Scheduled working days Mondays and Fridays with the ability to cover staff vacations/time off 

  • Willingness to travel and transport clients

Qualifications 

  • Active and unrestricted LPN license (Massachusetts). 

  • Minimum 10 years of experience working with elderly populations. 

  • Background in long-term care, home care, or geriatric nursing required. 

  • Excellent communication and interpersonal skills. 

  • Strong attention to detail and organizational skills. 

  • Reliable transportation with valid insurance. 

  • Current CPR certification and up-to-date professional documentation. 

  • Ability to work both independently and collaboratively in a dynamic environment. 

  • Comfort adapting to changing patient needs and assignments. 

  • Experience with electronic medical records (EMR) is a plus.
